Information Security coverage for liability claims arising out of the activities performed by your practice to protect against the following:

• unauthorized access to and unauthorized use of it’s computer systems
• theft or destruction of data
• hacker attacks
• denial of authorized users to access computer systems
  
• failure to prevent the participation of a computer system in denial of service attacks
• failure to prevent the transmission of virus’ and other malicious code

Privacy Liability coverage for liability claims arising out of the following
privacy perils:

• disclosure of personally identifiable information and confidential corporate information
• violations of privacy policies
• disclosure of protected information pursuant to the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), the Gramm-Leach-Bliley Act of 1999 ("GLB"), the Federal Trade Commission and similar state based privacy laws. Coverage also includes the failure to protect personally identifiable information that results in identity theft (including claims for failing to disclose or warn of an actual or potential identity theft).
  

This coverage is written under the surplus lines law by Certain Underwriters at Lloyd's.  Underwriters at Lloyd's are not licensed insurers in any state except Kentucky, the United States Virgin Islands, and Illinois (where Underwriters are both licensed and eligible surplus lines insurers).  Underwriters at Lloyd's are eligible surplus lines insurers in all other states.
